Acute Rehab Unit (ARU) Care & Discharge Planner - Full Time, Days (Culver City)

The ARU Care and Discharge Planner, under the direction of the Case Manager, coordinates post-acute patient level of care needs (SNF's, Sub Acute, LTACH, Home Health Agencies, DME, etc) to facilitate a safe discharge of hospital patients, including barrier patients, to the appropriate setting, assuring continuity of care.
Responds effectively to questions and concerns expressed by patients and other staff and resolves identified problems.
Refers complex psychosocial issues to social workers.
Ensures documentation and technical compliance for discharge planning by coordinating with the clinical team.
Ensures timely and accurate completion of IRF PAIs for all patients.
Performs basic reception functions to ensure smooth department operations.

Now at 420 beds, SCH-CC is a general acute care hospital that utilizes many of the latest medical treatments and technologies.
The hospital offers a wide range of inpatient and outpatient acute care services, including an orthopedic center, cardiovascular services, acute rehabilitation, sub-acute care, psychiatric care and chemical dependency programs.
Additionally, SCH-CC operates a 24-hour emergency services center, which serves as a paramedic receiving station and is staffed by board-certified emergency physicians and nurse specialists.

placements, SNF's, Sub Acute, Home Healthcare, DME, LTACH.
Coordinates the progress of the discharge services requested with case managers, patients, families, physicians, other care providers and vendors to facilitate discharge plan and find necessary resources.
Notifies receiving facility of insurance information as needed, and processes referrals post-discharge.
Effectively communicates on the progress of the discharge services requested.
Appropriately schedules, reschedules, cancels or confirms patient appointments in a timely manner to maximize scheduling for patient volume and in accordance with established authorizations.
Conducts and documents post-discharge follow-up calls with patients and family members.
Collects and enters appropriate data required for completion of most current IRF PAI in a timely manner.
Performs basic reception functions to ensure smooth office operations, including answering incoming calls, maintaining an accurate filing system, copying forms and documents, etc.
Effectively uses computer programs to obtain demographic information and register patients, accurately verifies insurance coverage, including Medical Group eligibility.
Accurately collects and documents all necessary and required information and activities.
Effectively communicates with patients and physicians to assure continuity of care.
Appropriately provides patients with written and verbal instructions of physician's orders and requirements for pre-operative diagnostic -testing and/or follow up appointments.
Coordinates ongoing team conferences during patient stays.
Appropriately determines patient eligibility in relation to established program, insurance or contract standards.
Responds effectively to questions and concerns expressed by patients and other staff and resolves identified problems (registration, eligibility, billing or financial) in a timely manner.
Acts as Data Steward by following appropriate procedures regarding requests, access and disclosure of PHI in accordance with HIPAA regulations, and takes necessary precautions to prevent unauthorized access.
Accurately prepares reports (volume and tracking) as needed or requested.
Minimum Education:
Bachelor's Degree in Social Work (BSW) or related degree in human services required.
Minimum
Experience:
Computer Proficiency and Literacy required.
Familiarity of Joint Commission, CMS, CDPH requirements required.
Excellent written and verbal communication skills in English required.
Ability to establish and maintain effective working relationships across the organization required.
Ability to facilitate and lead interdisciplinary rounds required.
Ability to multitask and maintain a work pace appropriate to workload required.
Previous experience in an acute care setting preferred.
